From analytical development to clinical implementation: a multiplex LC-MS/MS method for antihypertensive drug monitoring in resistant hypertension

Treatment adherence remains a major challenge in the management of resistant hypertension, creating a strong need for reliable analytical tools to monitor antihypertensive medications. This presentation describes the development and validation of a comprehensive LC-MS/MS method on the Shimadzu LCMS-8060NX platform for the simultaneous determination of a broad panel of antihypertensive drugs in both plasma and urine. The talk will discuss the analytical challenges associated with chemically diverse compounds and present the workflow for mass spectrometric and chromatographic optimization. Validation results obtained according to ICH M10 guidelines will demonstrate the method's sensitivity, precision, accuracy, robustness, and suitability for routine clinical use. Finally, real-world applications in nephrology, including inter-laboratory evaluation and patient result interpretation, will be presented, highlighting the potential of LC-MS/MS to support therapeutic drug monitoring, assess treatment adherence, and improve personalized hypertension management.
